    The role

    We’re looking for someone to join our trusted team to deliver a service that brings residents of Stoke-on-Trent safely home from hospital. You’ll be based at Royal Stoke University Hospital working with partners and establishing links within the voluntary and community sector to support customers and their needs.

    Job requirements

    • Support customers with their urgent practical needs, which are preventing them from being discharged home from hospital like providing emergency food, fitting temporary key safes, and arranging furniture moves.
    • Help to prevent hospital re-admission by completing referrals to the ‘Voluntary Community and Social Enterprise’ support services or to social prescribers.
    • Record patient engagement, completing monitoring as required to track service effectiveness.
    • Balance the needs of the service with the needs of the customer by taking a flexible approach.
    • Have a great partnership approach by working with staff to ensure the service is embedded and continues to meet the needs of the community.
    • Promote the service to ensure customers who need it can access it by engaging with VAST and other local directories.

    What we’re looking for:

    • Experience of working in the Social Care sector.
    • Excellent communication skills.
    • An understanding of what great customer service is.
    • A commitment to fulfilling contractual requirements for commissioned services.
    • Capability to work under pressure and manage multiple tasks and demands.
    • Idea generation and implementation.
    • Capability to report both to your line manager and the service manager for regular updates.

    We’re looking for someone to join our trusted team to lead and develop a service that supports residents in North Staffordshire to return safely home from hospital. You’ll be based at Royal Stoke University Hospital working out of the Integrated Discharge Hub leading a small team, coordinating a strategic approach, and building great partner relationship to ensure the services’ success.

    Job requirements

    • Manage a small team of coordinators by providing day-to-day support, one-to-one supervisions, check-ins, and team meetings.
    • Provide guidance, share best practices, and act as the main point of contact for new developments or changes.
    • Coordinate with the service manager and the wider team for seamless integration and streamlined communications.
    • Collect quarterly monitoring data, set out outputs and achievements, and attend project review meetings.
    • Support your team with any patient resources and requirements for building a ‘Voluntary Community and Social Enterprise’ support directory.
    • Lead on engagement with VAST and other local directories to ensure awareness of local support services.
    • Contribute to addressing community needs through the ‘Voluntary Community and Social Enterprise’ sector development activities.

    What we’re looking for:

    • A degree level education or substantial experience in this area.
    • Excellent communication skills.
    • An understanding of what good customer service is.
    • A commitment to fulfilling contractual requirements for commissioned services.
    • Idea generation and implementation.
    • A motivational approach to help lead, coach, and support team members.
    • A flexible approach that meets service and team demands.
    • Capability to work under pressure and manage multiple tasks and demands.
    • Capability to report both to your line manager and the service manager for regular updates.

    We’re looking for someone to join our trusted team on fixed-term 6-month basis. You will be responsible for supporting all Revival services by taking referrals by phone, email, setting up customer cases, and booking in appointments for colleagues.

    Job requirements

    • Take referrals and deal with all enquiries from the Revival mailbox.
    • Answer phone calls on our internal call system.
    • Add cases on to our case manager system.
    • Be responsible for cases and adding appointments, costs, and interventions where necessary.
    • Be the first point of contact for many of our referrers.
    • Support colleagues when necessary and working to a ‘one team approach’.
    • Send out customer feedback forms and adding responses to our internal database.
    • Manage incoming and outgoing daily post.

    What we’re looking for:

    • Experience in a similar role, or of completing admin tasks including scheduling appointments for several members of staff.
    • Excellent communication skills.
    • Ability to fulfil contractual commitments for commissioned services.
    • Ability to work under pressure and manage multiple tasks and demands.
    • Excellent ICT skills.

    We’re looking for someone to join our trusted team to supervise our Hospital Discharge and Welfare service. You and the team will support customers being discharged home from hospital to stay safe and well in their homes.

    Job requirements

    • Manage a team of support workers, providing day-to-day support one-to-one supervisions, check-ins and team meetings.
    • Provide guidance, share best practice, and act as the main point of contact for service changes or developments.
    • Create and manage a 7-days-a-week rota ensuring the service is staffed effectively.
    • Support customers by completing holistic assessments and agreeing an action plan to keep them safe, warm, and well at home.
    • Complete practical tasks to support customers at home like completing property inspections, arranging deep cleans, and completing check-ins.

    What we’re looking for:

    • Experience of working in a similar role, or health and social care setting. Or qualified by having knowledge, experience and a demonstratable attitude to support older or vulnerable people.
    • Excellent communication skills.
    • Ability to work with vulnerable or older people.
    • An understanding of what great customer service is.
    • Ability to fulfil contractual commitments for commissioned services.
    • Ability to work under pressure and manage multiple tasks and demands.
    • A motivational approach to help lead, coach, and support team members.
    • Excellent ICT skills.
    • Ability to work weekends and bank holidays on a rota basis.
    • Ability to be on call in the event of an emergency for working staff on a rota basis.
