At Revival, our focus is always on helping people feel safe, supported and able to live independently at home. Over the past month, our teams have continued to support people through a wide range of challenges, often at times when life feels uncertain or overwhelming.

Across our Derbyshire low level support service, wellbeing support service and VCSE hospital discharge team, we continue to provide high levels of preventative and responsive support.

Many of the people we support are facing complex situations, including returning home from hospital, living in unsafe environments, experiencing loneliness, managing financial pressures or coping with changes in their health. Our role is to step in at the right time, helping to reduce stress and create stability.

Helping people return home safely
Our VCSE team continues to work closely with hospital discharge teams to support people returning home safely and comfortably.

Between January and March, the team supported 186 people and delivered 478 hours of support. This included time spent on hospital wards, carrying out property checks, arranging essential items such as beds, heating and food, and coordinating with health and community services.

This support helps people leave hospital with confidence, knowing their home is ready and the right support is in place.

Making everyday life more manageable
Our wellbeing support service has supported 576 people over the past year, delivering 3,728 hours of support.

This includes welfare checks, collecting medication, supporting people to attend appointments and helping with everyday tasks like managing correspondence, finances and housing issues. By working alongside people in a calm and supportive way, the team helps make daily life feel more manageable and less overwhelming.

Supporting independence in Derbyshire
In Derbyshire, our low level support service has supported 67 people over the last three months, each receiving up to 12 weeks of tailored support.

Through home visits, help with benefit forms, housing applications and signposting to local services, the team supports people to overcome barriers and feel more secure in their own homes.

A focus on prevention and early support
Across all services, there has been a strong focus on preventing situations from escalating. This includes helping to avoid delayed hospital discharges, reducing the risk of readmission and supporting people at points of crisis, particularly when other services may not yet be in place.

Our teams have delivered hundreds of hours of preventative support, including arranging essential household items, supporting home set-ups and helping people access services they may not have been able to engage with before.

Continuing to improve how we support you
We’ve continued to strengthen how we deliver our services, ensuring a consistent and timely response for everyone referred to us. This includes maintaining 100% compliance with response and assessment times for both urgent and standard referrals.

We’ve also introduced improvements to make our processes more efficient, helping us respond more quickly and effectively to the people who need us.

Real moments that matter
Behind every number is a person, and we continue to see the real difference this support makes.

Recently, we’ve supported people to access financial support, including back payments that have allowed them to purchase essential items and improve their independence. We’ve helped individuals return to safe home environments so they can receive the care they need. We’ve also been there during incredibly important moments, ensuring someone had the right support in place at the end of their life.

We’ve also seen the positive impact of our befriending service, with one person now taking steps to become a befriender themselves after receiving support.

Listening to what matters
The feedback we receive continues to reflect the importance of this work. People often tell us they feel more at ease and reassured, with many describing a “weight lifted” once support is in place.

We regularly hear how much it means to have someone there to help, with comments about the kindness, compassion and consistency of our teams. Families also share how reassuring it is to know their loved ones are supported.

Working together in the community
We continue to work closely with health services, adult social care and voluntary sector partners to make sure support is joined up and accessible.
