Revival’s Reconnect Service: helping over 55s in Cheshire East reconnect with their local community

Jennifer came to the service because she felt isolated at home and had been battling with her mental health. Alongside that, her mobility had reduced and being alone at home for long periods of time was having an impact on her overall wellbeing.

Community Home Support Worker Sarah stepped in to help Jennifer feel more confident again, she explains.

“When Jennifer came to Revival she had lost her confidence, felt isolated and didn’t feel able to go out into her community alone.

“It was having a knock-on effect and started impacting her mental health and overall wellbeing.

With Jennifer's help Sarah put a plan in place to help her live independently in the community.

“To help reduce Jennifer’s loneliness and isolation I referred her to a local telephone befriending service. Where she could chat to someone, talk through her worries to help her feel less alone.

“I referred her to an NHS Wellbeing Hub for therapy to help with her depression and anxiety.

“To help her feel confident when out of the house, I helped her attend a coffee morning.

“Her reduced mobility was having an impact on her self-care and the upkeep of her home. I provided information on cleaning services and made a referral to Occupational Therapy for grab rails throughout the house.

“Because of Jennifer’s reduced mobility I helped make getting her prescription easier. While also arranging hospital transport for her appointments too.”

Thanks to Sarah's help Jennifer is now re-connected to her community.

“I’ve been able to give Jennifer the tools she needed to be part of her community and she now feels more like herself again.”
